Incoming Students

In order to register as a new student at Mokapu, please bring the following materials with you:

Welcome to Mokapu Elementary School
Our goal at Mokapu is to ensure that your transition into Mokapu is as smooth as possible.
We hope this information is helpful to you in the registration process.
REGISTRATION HOURS: Monday - Friday @ 8:30 am to 12:30 pm.
Documents needed for registration.
- Official birth certificate (or Passport, Baptismal certificate, Dependency Application).
- Medical records: State of Hawaii requires that all students complete a physical examination and a tuberculosis clearance within one year prior to entry into the school. Once you are in Hawaii, please ask your doctor/clinic to complete a Form 14 for you (this is your child's immunization record).
- Name and address of last school attended.
- For students receiving special education services please bring the current Individualized Education Program / Evaluation Data.
- For students with any special conditions of which the school should be aware (medical, legal, etc.), please bring documentation for verification of condition(s).
- Other relevant information, such as home address, work/home phone number, social security card, unit name, emergency contact information, etc.

- Please ensure all accounts are settled:
- Clearance with Meal Account Clerk for any outstanding meal loans.
- Clearance with Library for any outstanding library books.
- Clearance with Teacher for any outstanding text books.

1. Notice of Request for Accommodations
The School will provide notice of the availability of ADA access and accommodations in forms including, but not limited to, a “Notice & Request for Accommodations” flyer made available to all students, event registration forms, season schedules for school athletic events, and other school event announcements. This notice will include the International Symbol of Accessibility (“ISA”), contact information, and language concerning ADA access and accommodation. (See Appendix A)
2. Signage
The School will work toward providing signage at all inaccessible entrances to each of its facilities, directing users to an accessible entrance or to a location at which they can obtain information about accessible facilities.

In accordance with the Americans With Disabilities Act (“ADA”). Mokapu Elementary School (“the School”) is committed to ensuring that individuals with disabilities are not denied an opportunity to participate in and benefit from any service, program, or activity, offered by the school. Individuals with disabilities are welcome to participate in and benefit from the events at our school; School officials wish to assist in making the event a memorable experience for all.
ACCESS TO SERVICES, PROGRAMS, AND ACTIVITIES
The School will make reasonable modifications in its policies, and procedures when the modifications are necessary to ensure that services, programs, and activities provided by the school are readily accessible by individuals with disabilities. In choosing among methods to ensure that services, programs and activities are readily accessible, the school will give priority consideration to those methods that will provide the services programs, and activities, in the most integrated setting appropriate to the needs of individuals with disabilities.
FACILITIES ACCESS
Although not required for compliance with the ADA, the School has elected to take steps to modify it’s existing facilities through additional barrier removal, such that individuals may have more access to school facilities, The school has a transitional plan which outlines the physical barriers of all the sites where programs, services, and activities are held, a plan prioritizes the removal of those barriers with cost estimates. The Department of Education’s (“DOE”) Facilities Branch is possession of the transitional plan.
These additional structural modifications are done in compliance with the Americans with Disabilities Act Accessibility Guidelines for Buildings and Facilities (“ADAAG”).
NOTICE & REQUEST FOR ACCOMMODATIONS
If you are an individual with a disability, please contact the following personnel to make arrangements for accessibility to any school event at least 10 working days prior to the event. Reasonable efforts will be made to accommodate your request.
